Just a quick note that I'm not ignoring this. I have to set up a way to provide access without opening up everything I have online. I hope to do this over the weekend!

No problem. All I really need it FTP and Pixelpost admin access for now.
Anyway, while I wait, you can provide me with some pertinent information.
For starters:
Your hosting provider (GoDaddy, Eleven 2, etc)
Hosting type (Linux, Windows, etc)
PHP version (If you don't know it, don't worry)
MySQL version (If you don't know it, don't worry)
How often does it occur?

1: Dreamhost
2: Linux
3: 5.2.x
4: not sure
5: varies. it tends to mostly happen for me when I update add-on code on add-ons that are already installed.

Was there ever any resolution to this issue?

Perhaps my earlier post was a little bit too brief, especially for a first post, aka, forum loafer. I am experiencing this addon behavior regularly, i.e. a couple of times a day I log into the admin panel to re-enable the 'Promote' and 'Top Commenters' addons. I've tried deleting all unused addons to no avail. Any help would be very much appreciated.

I've been suffering this for the last couple of days with a couple of custom addons I've written. In the end I've moved the addon code directly into the index.php file in the root to resove the issue. While this is a little bit of a sledge hammer approach to the problem, it does mean consistency for visitors to my site.

I would like all having this issue to try something for me.
Goto your pixelpost admin/ folder and delete the following files and folders.
install.php (file)
install/ (folder)
Do not worry, your blog will still function as intended with both file and folder removed.

I'll restore my addons back to the addon directory and see what happens.

I was having this problem as well. However, when I moved pixelpost down to my root directory the problem resolved itself. I moved only the necessary files so install.php and install/ were not put on the root. This could be what fixed it for me.

I removed install/ and install.php, and the problem is still occurring for me.
